Four SwedishAmerican Medical Group clinics have received prestigious Level 3 Patient-Centered Medical Home recognition from the National Committee for Quality Assurance (NCQA).

NCQA?s Patient-Centered Medical Home (PCMH) is an innovative program for improving primary care. In a set of standards that describe clear and specific criteria, the program gives practices information about organizing care around patients, working in teams, and coordinating and tracking care over time.

The PCMH is a health care setting that facilitates partnerships between individual patients, their personal physicians, and when appropriate, the patient?s family.

Care is facilitated by registries, information technology, health information exchange and other means to assure patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.

The following four SwedishAmerican Medical Group clinics received Level 3 recognition (the highest recognition status obtainable):

? SwedishAmerican Medical Group ? Davis Junction Clinic, Davis Junction;

? SwedishAmerican Medical Group ? Five Points Clinic, Rockford;

? SwedishAmerican Medical Group ? Valley Clinic, Rockford; and

? SwedishAmerican Medical Group ? Woodside Clinic, Rockford.

From a maximum possible score of 100, 75 points are needed to receive Level 3 recognition. Each SwedishAmerican clinic received a score of 87.25. The recognition places these clinics and their respective providers among an elite group.

Approximately 15,000 primary care doctors practice in Illinois. NCQA has recognized 415 physicians and locations for any of their certifications. Of these, 256 are Level 3 PCMH, and 23 of them are SwedishAmerican Medical Group physicians.

?This is a prime example of how we are coordinating care, reaching out to patients and trying to get ahead of the curve, rather than being reactionary in health care,? said SwedishAmerican Medical Group President Dr. Thomas Schiller.

Moving forward, SwedishAmerican plans to apply for Level 3 status for all of its clinics and their providers.

SwedishAmerican is a not-for-profit, locally governed health care system, with headquarters in Rockford. The health system is composed of two hospitals, 30 clinics, a home health care agency, a foundation, and has an exclusive affiliation with UW Health in northern Illinois.

While traveling through Europe on a motorcycle in the summer of 2010, I felt immersed in the beauty of the place; the mountains of Switzerland, the villages of Austria, the fields of Slovenia, and the beaches of Croatia. They all had their own character and beauty, but what amazed me the most was Bosnia. Seeing the mortar scars, shells of burnt out buildings, and monuments to the slaughtered was overwhelming and powerful, but it was balanced by beautiful strength and simplicity of life and community.

Many rural homes destroyed during the war had begun to be rebuilt by the residents by hand. They were amazingly crafted testaments to home and family, but also interesting to me were the equally well tended gardens and crops surrounding the homes. These agricultural elements were not limited to the country homes; in a number of cities the housing towers were surrounded by the residents? gardens. And as I sat eating the roasted corn, tomatoes, and bell peppers grown by my friend?s family in the small garden plot in front of their apartment building, I couldn?t help but think that these humble Bosnian gardens could teach us Americans a lesson or two.

To many Americans urban agriculture seems like a pipedream or an upper-middle class hobby. Our Bosnian counterparts prove this wrong; but why can they do it while we feel it isn?t possible in the mainstream?

Education is one of the largest barriers?against the implementation of urban agriculture in the United States; many Americans don?t understand the variety of urban agricultural typologies that exist. These interventions can range in scale from a balcony garden to a multi-acre commercial farm to a garden on a restaurant rooftop; and all types have a place and purpose in the city. Urban agriculture doesn?t always imply large scale projects. As with the gardens in Bosnia, a large impact can made through personal small scale gardening.

But even more, urban agriculture can creatively take advantage of unused or under-utilized spaces. In fact, a vast majority of unused space in cities is in the public right-of-way ? consider the space under power lines and along boulevards. Some people have realized the potential of these spaces and begun planting gardens illegally, spurring the term ?Guerilla Gardening?. But also, gardens can be excellent interim uses for vacant plots. Not only do they aesthetically improve the area, but they are also a great tool to promote and inform the public about the potential of urban gardening.

Finally, regulations went a long way to deter agriculture in the city. Considering that zoning origins can be partially traced health concerns of city living, it is not surprising at some point we put a stop to agriculture in city limits. Luckily, cities such as Seattle have learned the errors of their ways and begun revising the zoning codes and ordinances to allow more flexibility for urban agriculture. In fact, King county employees has even begun an educational and charity urban garden in Seattle called Goat Hill. Government leadership is key to making urban agriculture a mainstream phenomenon.

Seattle has made its presence known in the urban agriculture revolution, but still has far to go. Subsidence agriculture in American cities is not impossible, and whether it is government or rebel lead, education is the key to invoking change. Our society has not only forgotten to eat our vegetables, but also forgotten how to grow them. It is ironic since the United States was found on agrarian principles. It is time we relearn our roots.

Real Estate Marketing ? How to Write Follow-Up Letters : Data List ...

Hopefully, your real estate website has a capture page for buyers to tell you what they?re looking for, and another capture page where sellers can get an idea of their home?s worth in the current market.

If you?ve taken it a step further, you also offer a special report to buyers or sellers or one for each.

That means you have the opportunity to follow up with a ?drip campaign? to stay in touch with those leads. If you do, and if you do it well, you?ll be the agent they call when they?re ready to take action.

It?s a fantastic opportunity, but too many agents are wasting it by sending letters that repel rather than attract.

Their letters say things like ?If you?d like to look at homes, call me.? Or ?I?m a really wonderful real estate agent, so if you?re ready to list, call me.?

Some agents send those same letters over and over and over. And what do consumers do? Most merely hit the delete button.

Instead, you need to be sending information they?ll be glad to get ? so that when they see your name in their in-box, they?ll open your message and actually read it.

What can you send instead?

Market information, for one thing. People who are interested in buying or selling are interested in what goes on with the local market. So give them statistics:

• How many homes have sold in the past month?
• Is this more or fewer than the month before, or this month last year?
• What has been the average price?
• Is this average higher or lower than the previous month, or this month last year?
• How does this compare to national trends?

Unless you?re writing to people who responded to a specific special report ? such as one for FSBO?s, Expired Listings, Short Sales, or First time buyers ? then your information has to be generic.

You can do that and still offer useful information. But you do need to separate buyers from sellers.

You can give general tips on buying ? such as paperwork to take along when meeting with a lender, getting ready to view homes by narrowing your ?want? list, etc.

And you can give general tips on selling. That opens you up to everything from proper pricing to staging the house, to the reason why sellers should be absent during showings.

This is a time to avoid specifics. Don?t assume that your reader is a first time buyer or a newlywed or a retiree. Doing so shows that you don?t know who they are and many will see it as insulting.

Finally, unless you know you are proficient in grammar and spelling, get someone to check your work before you send it. Misused words, misspellings, and garbled sentence structure will do you more harm than sending no letter at all.

Remember, everything you send represents you and your professionalism. Make it good!
